To help the clinic staff better serve your child, we ask that you fill out the yellow clinic card with all known health conditions, medications, allergies and contact information.

IF YOUR CHILD BECOMES ILL AT SCHOOL

Our clinic staff will do everything necessary to ensure the health and well- being of our students while they are at school. However, students sometimes become too ill to remain in their class. The nurse will assess the student to determine whether your child will need to be picked up from school. In order to maintain your child’s well-being, as well as the health of other students and staff members, we ask that you:

1.     Notify the school if your phone number has changed.

2.     Have a back up person available to pick up your child for illness if you are unable to.

3.     Make sure to keep the clinic card updated.

4.     Please do not send your child to school with a fever or while in the early stages of a cold/illness.  Pinellas County School Health Services recommendation is that a child should be fever free for 24hours without fever-reducing medication before returning to school.

5.     If your child has been hospitalized or has had surgery, a doctor’s note should be provided indicating that your child may return to school and restrictions, if any, should be noted.

6.     Please be aware that in the event of a medical emergency where paramedics must be called to the school a “Code Blue” will be activated. During a Code Blue staff, students and parents/visitors must clear the hallways and remain in their location until the code is cleared by administration.

MEDICATIONS AT SCHOOL

1.     If it is necessary for your child to take medicine at school, the medication must be brought to the clinic by the parent/guardian and signed in by the nursing staff.  A medication administration form must also be signed by the parent/guardian.   Students are not permitted to transport medications to and from school.  Medication is not to be given to the bus associate to deliver.

2.     Prescription medications taken at school must include an exact time (10am, 11am, etc.) the medication is to be given at school on the bottle label. The medication will not be accepted if it reads:  three times a day, four times a day, etc. ** Please ensure that your child’s physician specifies the time the medication is to be given at school on the prescription before taking it to the pharmacy.
