Life Insurance:

While no amount of income can compensate for the death of a family member, it is comforting to know that survivors are able to meet family financial obligations through a sound life insurance program.
Your Life Insurance Plan Includes:
- Basic Employee Term Life
- Optional Employee Term Life
- Optional Dependent Term Life (Spouse & Child[ren])
- Optional Family Term Life
Pinellas County Schools provides Basic Employee Term Life insurance coverage through The Standard Insurance Company. It is one times your annual base salary, rounded up to the next $1000, with minimum coverage of $15,000.
For example:Annual Salary Basic Coverage is:
$12,000
$15,000 (minimum $15,000 Coverage)
$25,000
$25,000 (1 X your annual base salary)
$27,500
$28,000 (rounded up to next $1000)
Pinellas County Schools also provides Basic Employee Accidental Death and Dismemberment Insurance of $2,000 at no cost for employees.
As a New Hire or during Annual Enrollment, employees can enroll in Optional Term Life for themselves, their spouse, child(ren) and family. Optional Term Life coverage provides options of up to $500,000 for yourself, $100,000 for your spouse and $10,000 for your dependent child(ren).Review The Standard Insurance Company Life and AD&D Insurance Plans and Rates.
Beneficiary Information:
A beneficiary is the person or entity who will receive the benefit in the event of your death. You may choose more than one beneficiary and designate them as primary or secondary beneficiaries. Your primary beneficiary is the first in line to receive your death benefit. If the primary beneficiary dies before you, a secondary beneficiary is next in line. Total percentage amounts for both primary and secondary beneficiaries must equal 100%.
You may update your life insurance beneficiaries at any time to ensure that your benefits will be accurately distributed by The Standard Insurance Company. To change or update your beneficiary information, you must complete the Benefits Enrollment & Change Form and submit it to the Risk Management Benefits Team:

Disabled Child Forms - Should be submitted prior to your dependent turning 26:
Eligibility of the dependent will be reviewed at the time of claim. In order to determine if your child qualifies to be covered, you may submit the form below and have The Standard Insurance Company determine if they are eligible after age 26.
